Human Services Awards Nominations

Each year, the Cobb Collaborative honors members of our community who have exhibited  outstanding service to those in need in Cobb County. Jack Vaughan, Jr. was a state  representative and member of the Cobb Collaborative who showed unselfish dedication  to public service and strongly advocated for those less fortunate in our community. In  memory of Jack’s service, the Collaborative honors a volunteer and a professional who  have gone above and beyond to serve those in need. 

We know that there are many individuals who have gone “above and beyond” this year  (or who have continued to go above and beyond!) in response to the varying needs of our Cobb County community. Please do your best to select one person from your organization who has demonstrated exceptional service and  leadership. 

Below you will find nomination forms in both the professional and volunteer categories.

Please note:

  • Individuals who have been nominated/honored in the last three years are NOT eligible for nomination in 2024.
  • Nominees must be employed or volunteer with a Cobb Collaborative member organization.
  • Agencies are welcome to submit in both the volunteer and professional categories. As stated above, please do your best to limit nominations to one per category. 
  • 2024 nominations are due no later than 5:00PM EST on Friday, October 25, 2024.

Jack Vaughan, Jr. was a state representative and member of the Cobb Collaborative who showed unselfish dedication to public service and strongly advocated for those less fortunate in our community. In memory of Jack’s service, the Collaborative annually honors both a volunteer and a professional who have gone above and beyond to serve those in need. Please use this form to nominate individuals in the PROFESSIONAL category.

Individuals who have been nominated/honored in the last three years are NOT eligible for nomination in 2024. Additionally, nominees must be employed with a Cobb Collaborative member organization.
